If Yes, register Lower Moderate Disability and Skip to end of form and record GOS-E Peds Score = 4. If No, go to 4bii. Data submission guidelines: If GOSEPCrrntWrkSchlInd=No, make sure that either GOSEPWrkSchlRestrictInd does not set to No or GOSEPWrkSchlReducedCapacInd does not set to No. Question 4a is Can the child function at work or in school at his or her previous capacity? If the answer is No (GOSEPCrrntWrkSchlInd=No), then the child must have some level of restriction. If the child has some level of restriction, then either Q4bi (GOSEPWrkSchlRestrictInd) or Q4bii (GOSEPWrkSchlReducedCapacInd) must be Yes. If both had a no answer, then the child would have no restriction, which would contradict the no answer given to 4a.
