The NIH Toolbox Empathic Behaviors Survey is a parent-report measure for children ages 3-12 that assesses parent perceptions of children's prosocial behaviors using a 10-item fixed length form. Each item administered has a 5-point scale with options ranging from never to always. Higher scores are indicative of more parent-reported child prosocial behaviors.
