The NIH Toolbox Friendship Survey is a self-report measure that assesses perceptions of friendship, using an 8-item fixed length form for ages 18-85 and a 5-item fixed length form for ages 8-17 Each item administered has a 5-pointScale with options ranging from never to always. Higher scores are indicative of a greater perceived availability of companions with whom to interact or affiliate.
