The NIH Toolbox Meaning and Purpose Survey is a self-report measure that is administered only to ages 18-85, as a computer-adaptive test (CAT). In addition, a fixed length self-report form is available for ages 18-85, as a supplemental measure. Items administered use a 5-point scale, with options ranging from strongly disagree to strongly agree or from not at all to very much. Higher scores indicate more self-reported meaning and purpose.
