The NIH Toolbox Perceived Rejection Survey is a self-report measure that assesses perceptions of rejection using an 8-item fixed length form for ages 18-85 and a 5-item fixed length form for ages 8-17 Each item administered has a 5-point scale with options ranging from never to always. Higher scores are indicative of greater perceived rejection.
