The NIH Toolbox Social Withdrawal Survey, an analogue to the NIH Toolbox Loneliness Survey, is a parent-report measure for children ages 3-12. It is a 4-item fixed length survey that assesses how often a child avoids or withdraws from social activities with peers. Each item administered has a 5-point scale with options ranging from never to always. Higher scores are indicative a higher level of social withdrawal.
