Teen/Young Child/Child report: Put a mark on the line that best shows how you feel now. If you have no pain or hurt, you would put a mark at the end of the line by the happy face. If you have some pain or hurt, you would put a mark near the middle of the line. If you have a whole lot of pain or hurt you would put a mark by the sad face. Parent report: Please rate how much pain you think your teen/child is haveing at the present time by placing a mark somewhere on the line.
