Document information on a continuing basis and check/update on discharge/death from review of medical charts. Add date stamp for when assessed. SCI: For each type of surgical procedure on the urinary/gatrointestinal tract indicate if it was performed SMA: Additional lines and other surgeries should be added as appropriate TBI:Choose all that apply. In previous trials and studies, information on surgical procedures has typically been documented in free text format, thus often precluding any meaningful analysis.
