The Washington University Clinical Dementia Rating (CDR) is increasingly used in longitudinal studies and clinical trials for staging the severity of Alzheimers disease (AD). The CDR is derived from a semi-structured interview with the patient and an appropriate informant and rates impairment in each of six cognitive categories (Memory, Orientation, Judgment and Problem Solving, Community Affairs, Home and Hobbies, and Personal Care) on a five-point scale in which none = 0, question-able = 0 5, mild = 1, moderate 2, and severe = 3. (Note Personal Care has no questionable impairment level.
