Description:
The SMFQ uses child and parent forms to assess and screen for symptoms of depressive disorders. The SMFQ is 13 items and asks about the child/ adolescent?s feelings and actions over the past two weeks. The SMFQ is appropriate for children ages 6-17. The SMFQ takes under 5 minutes to administer with forms for child and parent.The MFQ consists of a series of descriptive phrases regarding how the subject has been feeling or acting recently. Codings reflect whether the phrase was descriptive of the subject most of the time, sometimes, or not at all in the past two weeks.
